Enough is enough - insurers need to pay for standard treatments prescribed by the physician.  Insurers cannot cherry pick the evidence based guidelines they think a patient should be treated by.  The physician is the one licensed to make that determination for the patient.  CDC studies have demonstrated that most physicians ARE treating longer than the shorter-term recommendations insurers cover today.  So - long-term treatment IS the standard of care, and yet insurers are denying coverage calling such treatment "experimental".
